Carterknowle & Dore Medical Practice would like to invite you to join DoctorLink.

We have been looking at how we can improve access to Practice services; DoctorLink is our new online service where you can access advice and appointments online.
Registration is simple and you can register here or click the phone on the left.

For all urgent appointment, we ask that you undertake a symptom assessment through DoctorLink.

The symptom checker will ask you a number of questions to help to identify your problem.

The outcome of your assessment will direct you to the right treatment, whether emergency care, your GP, nurse or self-care.

If your outcome indicates that an appointment is required here, you simply click on the “book an appointment” button and book your appointment directly.

If no appointments are available, you will see a “request an appointment” button, please click this.

Once the request has been submitted, you will then be contacted by the Practice Team, to arrange an appointment within the identified timeframe (as outlined in the outcome of your symptom checker).

The information you provide will be shared with our clinical team (where appropriate) and will assist in providing robust information of your problem to the required clinical team member prior to your appointment.

If you wish to, you can now use the internet to book appointments with a GP, request regular medication, view a summary or look at detailed coded information from your medical record. You can also still use the telephone or call in the the surgery for any of these services.

The online service is available 24 hours a day, 7 days a week. The website has been fully tested and has been accredited as secure.  The availability of this service is governed by the practice and is dependent upon responsible use by you, the patient.  If not used responsibly, access to the online services will be revoked by the practice. 

Being able to see your record online might help you manage your medical conditions.

To gain access to the online system, login details will be required. If you would like access to the online services please complete the application form (link below), including your personal details and access type you require. Once you have read the information in the pack and completed the form, please bring the form to reception along with two forms of identification (one being photographic). Our receptionist will then issue you with login details- this will allow you to arrange appointments and order medication. If you request access to detailed coded information, this access is normally granted within 14 days - your request will be reviewed by a clinician at the practice to ensure that it is not deemed that access could cause physical and/or mental harm to the patient. Once access is granted patients will be notified via email.

Should you already have access to the online system but wish to request access to detailed coded information (from 1 April 2016) please complete the application and follow the procedure outlined above.
