Patient Registration
To register at the practice you will need to attend the surgery to complete some forms.
If you wish to register at the practice, please check the map on the next tab to see if your address is in our practice area, if you are unsure about this, call in or telephone the surgery to check this with us. When registering you may be asked to attend for a health screening appointment with a nurse or health care assistant. Please come to surgery some 15 minutes before your appointment in order to complete your registration and to fill in a brief health questionnaire.
We ask you to fill in this questionnaire because it can take a considerable time for us to receive your medical records. There is an online version of this form, which you may fill out and send to us.
Please note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
It is not currently possible to register with the practice online as your signature is required by the Health Authority on the registration form.
